What should I do if I do not feel well and your office is closed?

For emergencies, please contact University Police at (934) 420-2111.

For non emergent matters after hours, we have linked the following urgent care centers local to our campus:

If I feel sick, can I just go over the Health and Wellness Center?

Yes, we prefer you make an appointment, but if you feel sick and it is urgent, you can always come in during the day to be seen. Please log in to your patient portal to schedule any non urgent appointments at myhealth.farmingdale.edu

Covid-19

What do I do if I test positive for COVID-19?

If you test positive for COVID-19, stay home until you are fever free for 24 hours without the use of fever reducing medication and your symptoms are resolving.  

Once your symptoms have improved or resolved and you are fever free for 24 hours without the use of fever reducing medications, you can return to school and/or work. 

You may take additional measures for the next 5 days to curb disease spread, such as enhancing hygiene practices, wearing a well-fitting mask and keeping a distance from others.  These additional measures are encouraged.

What do I need to be in compliance with my health records?

Can I register for classes before I submit my immunization requirement?

No. You must submit you immunization requirements before you are able to register for classes.

When is my immunization requirement due?

You must submit your MMR immunizations and proof of the Meningitis vaccine (within 5 years), or a Meningitis Response Form before you register for classes. Resident students must submit proof of Meningitis vaccine within 5 years, a waiver will not be accepted.

I am a part-time student (This includes summer & intersession, up to 11 credits).

1. Proof of 2 measles, mumps and rubella (MMR) vaccinations. First dose is on or after your 1st birthday.

2. Meningitis vaccination within 5 years or a Meningitis Response Form, which can be found in Important Forms on our website. If you do not wish to have the meningitis vaccine, you can waive it on the Meningitis Response Form.

I am a full-time commuter student (12 or more credits).

1. Proof of 2 measles, mumps and rubella (MMR) vaccinations. First dose is on or after your 1st birthday.

2. Meningitis vaccination within 5 years or a Meningitis Response Form, which can be found in Important Forms on our website. If you do not wish to have the meningitis vaccine, you can waive it on the Meningitis Response Form.

3. Recommended- a complete health record found in Important Forms. A complete health record includes immunizations, laboratory testing and a physical exam.

I am a resident student.

1. Proof of 2 measles, mumps and rubella (MMR) vaccinations. First dose on or after your 1st birthday.

2. Meningitis vaccination within 5 years. Resident student are REQUIRED to have the meningitis vaccination. The meningitis vaccination can not be waived for resident students.

I am an athlete playing on a FSC team.

1. Completion of a Health Record found in Important Forms.  A complete health record includes immunizations, a physical exam and a Meningitis Response Form. Physical must be valid for entire school year in which you are participating, not just for your season.

2. Completion of a Cardiac Athletic Questionnaire.  Any "yes" responses will require clearance from a Cardiologist. The form can be found in the Important Forms section on our website.

3. Completion of the Sickle Cell Trait Status Verification Form (along with a copy of the lab results or newborn HgbAS screening result) for first year or transfer students.  

Please note: an updated athletic physical is required every year. The form for returning Athletes can be found in the Important Forms section on our website.

I am a nursing, dental hygiene or medical laboratory technology student.

1. Completion of a health record. A complete health record is required every year.

2. Titers MUST be submitted, with lab report, for the following: Measles, Mumps, Rubella, Varicella and Hepatitis B. 

3. All forms are to be submitted through Castle Branch system.

Note: 1st year Nursing and Medical Lab Tech students must also submit copies to the Health and Wellness Center.

*Dental Hygiene students must submit records to Health and Wellness each year.*

I am an online only (remote) student.

1. Submission of a completed Meningitis Response Form. (Can be found in Important Forms or can be completed via patient portal.) 

What is an MMR?

MMR stands for Measles, Mumps and Rubella. It is an vaccination most people receive in childhood. It is a series of 2 shots. The first shot must be given on or after your 1st birthday.

Where can I get a copy of my immunizations?

You can get these records from your pediatrician, high school or from a previous college you attended. If you are unable to get these records, you can have blood work drawn for MMR titers and submit the lab report showing immunity.

How can I submit my records?

  1. The most effective way to submit immunization records is through the student patient portal at myhealth.farmingdale.edu. This will allow the Health and Wellness staff to process your records in a timely manner.
  2. Fax your records to: (934) 420-2137.
  3. Email your records to: wellness@farmingdale.edu.
  4. You may bring them directly to the Health and Wellness Center. If the office is closed there are secure mailboxes at the front and rear of the building.
  5. Mail your records to: Farmingdale State College, Health and Wellness Center, 2350 Broadhollow Rd, Farmingdale, NY 11735.
Please make sure your name and RAM number appear on all paperwork.

How can I verify that you received my records?

If you uploaded your records via the patient portal, you can check your compliance by logging in to the portal, select the Required Immunizations and Forms tab on the left where you can view your compliance status. The status will show as compliant and satisfied if it is complete, if not, it will show as Not Compliant and either say No Data if no document has been submitted, or awaiting review, meaning it was received but needs to be reviewed by one of our staff members. 

You can also send a secure message via the portal to confirm we received your paperwork. 

If you faxed your records, please allow 48 hours for the records to be processed, then call the office at (934) 420-2009. If you mailed your records, please allow 7 business days before calling (934) 420-2009 for verification. 

Can I get my immunizations and Health Record done at the Health and Wellness Center?

Yes, you may have your immunizations and health record completed at the Health and Wellness Center. Please call (934) 420-2009 for an appointment or you can log in to your patient portal at myhealth.farmingdale.edu to self schedule your appointment.
