Admissions Forms

Select the below form based on your individual circumstance and goals. 

• Have you already applied for an upcoming term and would like to change your major? If so, please log in to your FSC Ram Portal to request a change of major. 
• Did you apply to FSC previously and you would like your application reviewed for an upcoming term? Complete the New Student Semester Change Form.
• If you are a current FSC student looking to apply for Nursing, Dental Hygiene AAS, or Dental Hygiene Completion programs, fill out the Current Student Curriculum Change Form.  
• If you are a current FSC student looking to apply to Applied Psychology, please fill out the Current Student Curriculum Change Form.
• Have you been accepted but do not plan on attending FSC? Please Withdraw Your Acceptance.
• If you'd like to appeal your application decision, please complete the Admissions Appeal Form.
