Veteran Student Enrollment Certification (Blue Card)

The electronic VA Student Enrollment Certification (Blue Card) is a more convenient way for you to submit your registration information every semester you intend to use your benefits.  For general information regarding VA Benefits, college polices please review the Farmingdale State College Student Veteran Benefit Handbook.  This handbook is updated frequently, so please check back for updates!

The Registrar’s Office is located in Laffin Hall room 225. Normal business hours: Mon-Fri 8:30-4:30. Summer hours are Mon-Fri 8:30 to 4:00pm.

We will also continue to service you via email, should you need to send an inquiry to regoffice@farmingdale.edu.

If you wish to contact our office by phone, you may do so by calling 934-420-2778. You should ask to speak with a Farmingdale School Certifying Official if you have any questions about your benefits.

If this is the first semester you are using your benefits at Farmingdale, you must speak with a Farmingdale School Certifying Official before completing the blue card. There are important documents we need from you and policies/procedures to discuss with you for your first semester.

Also, you are required to contact Farmingdale's School Certifying Official before completing the blue card every semester you plan on using your benefits, if you are in the Aeronautical Science: Professional Pilot program. A School Certifying Official will review important information with you regarding the VA approval process for flight fees.

You can only submit your VA enrollment certification once you are registered and have finalized your schedule.  If you anticipate making substantial changes, please wait to submit this form until your schedule is completed.

You must submit this form every semester you intend to use your VA benefits. Click here to be directed to the VA electronic blue card.
